Teva Support Solutions® is a comprehensive program, providing personalized support, training, and education to you and your healthcare provider. Our nurses offer support throughout your CINQAIR® (reslizumab) Injection treatment journey.

HOW CAN TEVA SUPPORT SOLUTIONS® HELP ME?

How can I use Teva Support Solutions®?


A:

A Nurse Case Administrator is specially trained to guide you through the insurance process and provide financial assistance options. Clinical Nurse Educators (CNEs) care about your treatment experience and can provide you with personalized support and education with you in your home or other location to help with questions about treatment. Contact Teva Support Solutions® at 1-844-838-2211 to learn if there is a CNE available in your area.

Are there websites I can visit or numbers I can call if I want more CINQAIR® or treatment-related information?


A:

Yes. You can visit CINQAIR.com or call Teva Support Solutions® at 1-844-838-2211 for information about your treatment.

Q:

I have concerns regarding my insurance coverage. Can you help answer my questions?


A:

At Teva Support Solutions®, we are happy to help. For any questions or concerns, please call a Nurse Case Administrator at 1-844-838-2211. Nurse Case Administrators are available Monday through Friday, 9 AM to 7 PM ET.

Q:

Is there a way to lower my out-of-pocket costs?


A:

We may be able to help you find programs to make treatment more affordable. If you have health insurance with coverage for CINQAIR® through a commercial insurance provider, you may pay as little as $0 out-of-pocket for the cost of CINQAIR® and its administration (up to $10,000).*

To access these and other benefits, ask your healthcare provider to enroll you in Teva Support Solutions®.

The CINQAIR® Cost Support Program helps commercially insured individuals in the United States (including the United States territories) who are prescribed CINQAIR® pay for their eligible out-of-pocket costs for the drug and for administration of the drug, including co-pays, co-insurance, and deductibles. The Program does not cover the costs of physician office visits or evaluations, blood work or other testing, or transportation. Persons residing in the states of Massachusetts, Minnesota, Michigan, and Rhode Island are eligible for financial assistance with their out-of-pocket costs for the drug only and are NOT eligible for other types of financial assistance (administration of the drug). Eligible individuals must have commercial insurance coverage for CINQAIR®. Uninsured and cash paying patients are NOT eligible for this Program, nor are individuals with commercial insurance coverage that does not provide coverage for CINQAIR®.

Is there assistance available if I have Medicare, Medicaid, TRICARE, other government insurance, or no insurance at all?

HOW DO I ENROLL IN TEVA SUPPORT SOLUTIONS®?

How do I enroll in Teva Support Solutions®?


A:

If you and your healthcare provider decide that CINQAIR® is right for you, be sure to enroll in Teva Support Solutions®. Work with your healthcare provider to complete the Prescription and Service Request Form and ask him or her to fax it in so that you can start receiving the benefits and support this program has to offer.
