What does a Summary of Benefits include?

A Summary of Benefits will include information about your patient's health insurance coverage, including if the health insurance plan requires more information before the patient can begin treatment. If so, Teva Support Solutions® may include the plan's prior authorization request form and submission instructions or the plan's policy.
